Authorization Form for using a Purchase Order

Please completely fill out this form and SUBMIT it in order to receive authorization to use a purchase order. to register for MaFLA event, you must be an active MaFLA member. Request to use Perchase Order must be received at least 2 weeks prior to the close of registration for the event.  MaFLA typically will respond to requests for Purchase Order usage within 72 hours of the receipt of this form.

PLEASE NOTE: It is the responsibility of each individual named on the purchase order to register or renew their membership on the MaFLA website even with the submission of the purchase order. MaFLA does not complete this action for registrants based on the submission of this form for membership or for events.  Each individual registrant must register for their own participation as a member or as an event participant. 

  • Purchase Orders cannot be used for late registration.
  • Purchase orders may not be used for Early Bird rates for MaFLA events.
